Certified Community Behavioral Health Clinic (CCBHC)

Coming Soon: A New Standard of Community Wellness

Impact Life is proud to announce our transition toward becoming a Certified Community Behavioral Health Clinic (CCBHC).

This evolution represents a major step forward in how we deliver mental health and substance use support—making care more accessible, more connected, and more effective for the entire community.

What is a CCBHC?

A Certified Community Behavioral Health Clinic (CCBHC) is designed to be a “one-stop shop” for behavioral health care. This model was created to:

Fill critical gaps in the healthcare system

Provide comprehensive, integrated services

Ensure access to care regardless of insurance or ability to pay

In short: it’s a better, more complete way to deliver care to those who need it most.

Our Vision and Goals

Our goal is to create a seamless, “no wrong door” experience—where anyone who reaches out can get the help they need.

We Aim to:

  • Integrate Care
Combine mental health, substance use, and physical health screening in one place
  • Increase Access
Remove financial and geographic barriers to treatment
  • Improve Outcomes
Deliver evidence-based, person-centered care that treats the whole person

Comprehensive services

As we build out our CCBHC program, we are introducing a full range of services designed to support every stage of care and recovery:

24/7 Crisis Mental Health Services

(including Mobile Crisis response)

Screening, Assessment & Diagnosis

Patient-Centered Treatment Planning

Outpatient Mental Health + Substance Use Services

Targeted Case Management

Primary Care Screening & Monitoring

(vitals related to mental health)

Psychiatric Rehabilitation Services

Peer + Family Support Services

Specialized Services for Veterans & Active-Duty Military

Who we Serve

The CCBHC model is built to serve individuals of all ages and backgrounds.

Our Focus Includes:

  • Adults with Serious Mental Illness (SMI)
  • Children and adolescents with Serious Emotional Disturbance (SED)
  • Individuals with Substance Use Disorders (SUD)
  • Individuals with co-occurring disorders (mental health + substance use)
  • Veterans and active-duty military members

Why This matters

For too long, behavioral health care has been fragmented and difficult to navigate.

People often:

  • Visit multiple locations for different services
  • Face barriers due to insurance or cost
  • Struggle to find coordinated, consistent care

The CCBHC model changes that.

At Impact Life, we are creating a system where:

Care is connected and collaborative

You are treated as a partner in your recovery

You are supported by a multidisciplinary team of providers and peers

Looking Ahead

This transition represents more than a new program—it’s a commitment to raising the standard of care in our community.

Better access. Better coordination. Better outcomes.
