IMPACT LIFE — EST. 2020

Social Prescribing +
Primary Care Program

Redefining recovery and wellness through whole-person, trauma-informed care.

This community-based initiative integrates primary care, behavioral health, nutrition, and social prescribing to bridge the gap between clinical treatment and everyday life. Through personalized wellness plans, participants engage in both clinical and non-clinical supports—from counseling and nutrition sessions to creative, movement, and peer-led activities—helping individuals restore health, connection, and purpose.

WHAT IS social prescribing?

Social prescribing connects people to non-clinical, community-based activities that support recovery and well-being. These include:

Gardening and art workshops

Movement and mindfulness classes

Culinary and nutrition sessions

Peer and creative expression groups

This approach addresses social and environmental barriers—like isolation, food insecurity, and stress—by promoting meaningful engagement and belonging.
